During this inspection it was found that a fire drill had not been documented as being completed monthly since May of 2025. This was corrected at inspection when a fire drill was completed.

During this monitoring inspection a few outlets were missing safety covers. This was corrected at inspection when they were properly installed.
A review of the "Employees Who Left Employment in 2025" data from January 20, 2026, determined the center had not reported the number of employees who left employment in 2025 by January 15, 2026, as required.
Based on findings obtained during the course of the abuse/neglect investigation, information was obtained showing that the operation was not using a medication log to show how/when/ and what medications were brought to the day care daily. They also had no report to indicate when, where, how, or dosage of medications as they were given to children.
