The rear access area of the playground contained hazardous items, including protruding metal sticks and broken concrete ground. These items were accessible to children in care, posing a serious injury risk.
It was noticed that the emergency medical information for all the children being transported to the center was not available on the van. NOTE: this was corrected at inspection. The director made copies of their enrollment forms and put them in the vehicle binder.
