The door separating the infant and preschool classrooms had a large gap beneath it. Preschool children were observed pushing small toys under the door into the infant room. An infant was observed retrieving one of the toys and was observed with the toy completely inside their mouth. The gap beneath the door allowed items to pass freely between classrooms, creating a potential choking hazard for infants.
