One out four caregiver records reviewed was missing the following annual trainings for the year 1/22/2025-1/22/2026: hazardous materials, building safety, and medication administration.
The diaper changing mat in the toddler room was torn with the interior padding exposed. Note: The diaper changing mat was replaced with a new one during the inspection.
