Records were evaluated for ten children enrolled and the following was determined: Two children needed parent signatures on their food allergy emergency plan. Three children needed an address for the emergency contact. One child enrolled needed an updated immunization record. One child enrolled has permission to participate in water activities however there was no indication if the child is able to swim without assistance. Records for two children enrolled did not indicate if there is permission to transport for field trips or emergency care.
