A review of the Employees Who Left Employment in 2024 data from January 21, 2025, revealed the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.
Based on the information gathered, a child was not supervised by a caregiver in the playground due to caregiver completing other work duties. This allowed the child to squeeze through the bars of the gate causing her to wander outside the operation unsupervised for approximately 10 minutes. Child was unharmed.
