Observed a child walk out of a classroom to use a bathroom in the hallway. One teacher left the classroom and closed the door behind them and walked to the front of the building leaving the child in the bathroom with no visial, or auditory supervision. Licensing spoke to the person in charge and they returned the child to the correct classroom.

I spoke with caregivers regarding checking the children if they start crying to ensure that no injuries occurred and to always notify the parents of minot injuries the day of the injury when the parent picks the child up.

While doing a walk through there was an infant in the crib sleeping with three boppy pillows hanging on the crib. There were also two infants sleep in restrictive devices. This was fixed when the infants were laid down in the cribs and the other infant was taken out of the crib with the boppy pillows.
