During a DFPS investigation, a caregiver was found to have picked up a child from their cot and slammed them back onto their cot resulting in the child sustaining an injury near their eye that required medical attention and liquid stitching.
A caregiver hired after September, 2023, did not have a pre-application affidavit. This was corrected during the inspection when the caregiver was provided the form to complete.
