During an investigation it was found that a child had an accident during naptime and was in the soiled clothes until 5:30 when the child was picked up. The child was in the soiled clothes for around 3 hours. I spoke with the director about ensuring that children are going to the bathroom before naptime and checked after nap is over.

During a walk through of the building, one classroom stated that they had 12 children but actually had 13 children. When licensing asked again how many children they had, they stated they had 12 children. Licening had the caregivers count the children again and then they realized that they had 13 children in care and one hadn't been signed in for the day.
