The operation had removed a half wall between rooms 4 and 5 to make one classroom and furniture was not set up for use. Room 2 was previously permitted for infant use and is now being used as an 18-24 month classroom. NOTE: Re measurements and an updated space sheet was completed during the inspection.
Medication given to a child in the infant room was not in it's original prescription box. Two medications the child receives daily was placed into separate unlabeled vials together to be administered by the operation.
