Report finding
Based on information obtained by DFPS, play yards were placed right next to each other and touching, giving infants the ability to reach in.
A review of the Employees Who Left Employment in 2024 data revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.
There is sufficient evidence to determine that as of the inspection conducted on 1/18/24, one child's immunization record on file at the operation was out of date and did not contain all required immunizations.
