The following hazards were observed on the outdoor area: grass needs to be cut, the plastic structure has dirt and mildew on it, there is a hole in the middle of the open play area, the border surround the structure is splintered with nails exposed and off to the side of the structure, there are fallen tree branches, rocks and other debris.
