The operation failed to timely provide written notification to licensing of a change in director. During inspection, the operation verbally notified licensing that the operation's site director is no longer working for the operation. The site director's last day of work was reported and confirmed to have been back in late January of 2022. Therefore, licensing was not notified in writing within the required time frame.
The operation's initial permit/license was not posted or on file during inspection. This was corrected at inspection after a copy of the initial permit/license was emailed to the operation for posting.
