There is a tree stump of approximately 4-7 inches in height in the playground. The surrounding area of the tree stump is uneven, making it a tripping hazard. The center's wall on the back of the building, on the side of the outdoor playground has a removable piece measuring 22" by 37." The piece was detached from the wall exposing the underground of the building to the children's playground. This posed a safety hazard as the area where the missing piece is located is within childrens' reach.

Children observed climbing furniture and hiding underneath. One child was observed feeding an infant child food that was on the floor.
