The operation has not done the annual certification in writing that they have reviewed each of the recall notices issued by the Consumer Product Safety Commission and ensured that there are no unsafe products in the center.
There was a tire swing at the operation that was hanging from a tree. The swing did not have the required 30 inch clearance between the seat of the swing and the support structure. The tire was able to run into the tree trunk when swinging. The director removed the tire swing during the inspection.
