Based on evidence obtained caregivers did not follow up after contacting a parent using Brightwheel messaging system to ensure the parent received the message regarding an injury. The follow up was not sent until 2.5 hours after the first message.

Supplemental Form 2936 created to show that the deficiency was corrected at inspection. This supplemental Form 2936 replaces the original Form 2936 left at the operation on 11/9/2021. During the walk through of the 2 year old classroom there was diaper cream in an unlocked cabinet accessible to children. This was corrected when the caregiver locked the cabinet.
