During the inspection outlets accessible to children in both the two-year old class and three-year old class were not covered by child safety covers. The Director did immediately locate new outlet covers to cover the outlets when the issue was brought to her attention.
The operation's back building suffered major damage during a January 2026 winter storm that rendered the building unsafe for human occupation. The operation did not report this until late February of 2026. The operation has now permanently closed the building.
Based on information obtained during a DFPS investigation it was determined that the director of the operation had knowledge of concerning behavior by a staff person and continued with that person's employment, even going so far as to rehire them after terminating them previously.
