-The door to the men's restroom was off the hinge. I observed nails sticking out of the area. -Tarp was observed protruding from the loose fill surfacing, presenting a tripping hazard.
Based on the information obtained, it was determined a caregiver responsible for a group of children ages 3-5 years old did not maintain supervision when a child was able to pull out another child's hair.
