The cushion was exposed on a soft cushion age appropriate chair in the PreK 5 room resulting in it not being able to be properly sanitized. This was corrected when the chair was removed. The rug in the 2 year old room had a lot of soiled spots on it. This was corrected when the rug was removed. There was a small hole in the flooring in the older 2 year old room exposing the concrete and accessible to children and that area was sharp to the touch. This was corrected when the area was covered. There was a sharp bent paneling out on the 12-18month old playground that was accessible to children. This was corrected by placing foam over it with tape. A broken toy was found on the infant playground that was accessible to children and sharp to the touch. This was corrected when the riding toy was removed.
