During the inspection it was found that there were electrical outlets missing a protective covering inside a classroom with children younger than 4 years of age present. This was corrected at inspection when outlet covers were put in.
There has been a change in the filing system at the school building (Travis Elementary), where the operation is located. The operation was unable to provide children's records for review at the time of this inspection. They will locate and update licensing to share these records for further review of compliance.

A person without an approved background check was found to be serving in a classroom as a caregiver to children. This person had an approved background check with Memphis ISD but did not have one through licensing. This was corrected at inspection when the individual left the head start classroom and will not return to serve as a caregiver in a classroom until an approved background check is received.
