During the investigation, interviews with caregivers and children were conducted and there was sufficient evidence to show that caregivers yelled at the children in care.
It was determined throughout the course of the HHS investigation that the operation failed to report a child injury to Child Care Regulation within the 48 hours. The injury occurred on 11/28/22. The injury was not reported to Child Care Regulation until 12/2/22, exceeding the 48-hour time frame.

Report finding
The incident injury report provided by the operation was incomplete. The second page of the incident report was not provided to Child Care Regulation reflecting the caregiver's/director's/parent's signatures.
It was found that a tracking system for singing children in an out was being completed daily, but not being completed correctly. The initials of the person signing in or out each child was missing. I spoke to the operation about what needs to be on the sign in/out sheet.
