In the playground area there were 2 hazards seen, a broken soccer goal, and a small broken Basketball hoop. NOTE: This was corrected at inspection when both soccer goal and basketball hoop were removed from the playground.

One out of the four employee records reviewed is missing several of the required trainings for the annual year 6/5/2023-6/5/2024. The missing trainings include at least one clock hour in the following areas: prevention, recognition, and reporting of child maltreatment; emergency preparedness; preventing and responding to emergencies due to food or allergic reactions; recognizing and preventing shaken baby syndrome and abusive head trauma; understanding and using safe sleep practices to prevent sudden infant death syndrome (SIDS); and understanding early childhood brain development.
Emergency medical transport and treatment authorization forms for each child being transported was not in the vehcile used to transport the children.
