I evaluated 10 children's records and observed the following: 1. No address documented for the emergency contact in 2 children's records. 2. No date of admission in 1 chiild's file. 3. No information indicating who the parents have authorized to pick up the child in 1 child's file. 4. No information indicating whether the child has special needs documented in 6 out of 10 children's records evaluated.
