Based on the evidence obtained during the investigation, a staff member failed to correctly verify the identity of a child resulting in the wrong child being released from care.
It was found during the investigation that an employee used inappropriate discipline on a child in care by grabbing them by the arm resulting in an injury that required medical treatment.
