Two classrooms with children under five years old were missing outlet covers that were in reach of children. This was corrected at inspection when caregivers put covers on outlets.
A caregiver was deficient in appropriate guidance and discipline after she was observed pulling a child by the arm in an attempt to stop the child from running around. It was also observed by another witness about same caregiver on a similar incident of pulling a child.
