Upon review of personnel files 3 staff were missing a copy of their photo ID indicating age requirements, 4 staff were mising proof of background check, and 3 staff were missing affidavit 2985.
It was determined throughout the course of the HHS investigation that child in care were not supervised by the caregiver when the caregiver was assisting another child in care in the adjacent restroom. A young child in care received a minor injury as a result of inadequate supervision.
