Specialist reviewed 3 children with expired immunizations records. Per Childcare facility handbook on page 59. The childcare facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form DH 680, or the Religious Exemption from Immunization form, DH 681.
