Report comments
There was no incident/accident report for the injuries on the child that occurred during at the family home day care. TA: incident reports need to be filled out and signed on the day of the incident. Child was withdrawn from the home day care and an incident report was never signed. Provider did email a copy of an incident report after site visit. Parents did not sign the incident report.
