Inspector notes
Licensing Specialist observe d that the operator failed to maintain and/or document an active Staff Credential Verification on the departments training transcript. The Staff credential expired on 12/10/2024, Owner informed Licensing Specialist that the application to renew the Staff Credential was submitted to DCF training website on 12/15/2024 and still pending. Owner has until 03/06/2024 to bring this standard into compliance Owner will inform Licensing Specialist once the Staff Credential renewal is indorsed on the DCF training transcript.
