FDCH/LFCCH Handbook, section 2.4.2 Transportation Log A log must be maintained for all children being transported in the vehicle or on foot away from and/or to the premises of the home. The log must be retained for a minimum of 12 months. The log must include each childs name, date, time of departure, time of arrival and the signature of the driver verifying all children were accounted for during the visual sweep.
