A child enrolled on 11-24-25 ( S.M. ) did not have a Student Health Exam on file. Obtain the physical exam and submit a copy to the License Board office upon receipt. The provider was left the following reminders for the drop-in children: J.M. - physical exam expired as of 2-16-26 ; N.M. - physical exam expired as of 2-16-26. All information must be obtained prior to the children returning for care.
