During the time of the inspection, the Licensing Specialist monitored 5 of the 5 childrens records. During the time of the inspection, the Licensing Specialist observed 1 child did not have a Student Health Examination/DH (Form 3040), or an equivalent health statement on file within 30 days of enrollment.
