At the time of inspection, the Licensing specialist observed the child care personnel listed on the supplemental did not have the Child Abuse and Neglect form signed annually. Staff 1 last signed the form on 4/8/2025. Staff 2 last signed on 4/10/2025. The operator immediately had the staff sign the form at time of inspection. TA was provided to ensure that the Child Abuse and Neglect form is signed annually by the due date.

At the time of inspection, the Licensing specialist observed the child care personnel listed on the supplemental did not complete exposure plan training annually. Staff 1 and 2 both completed the training last on 4/9/2025. TA was provided to ensure that the exposure plan training is completed by the due date and annually every year there after.
