Correction / follow-up note
Provider's son has a delinquency screening that expired on 2-5-23 and does not have a new one and is not screened in the Clearinghouse. Licensing Specialist informed provider that her son needs to either get another delinquency screening or be screened in the Clearinghouse. She stated that she will have him screened through the Clearinghouse within 10 days. Submit the screening and add him to the Clearinghouse employee/contractor roster. Please submit the roster to PCLB as well as a Attestation of Good Moral Character.
