- 1 child did not have a Certificate of Immunization in their file. Provider stated childs parent were not getting the child vaccinated. The Provider had a pediatricians letter stating the parent refused vaccinations. Discussed with Provider the need for a religious exemption form from the Florida Department of Health for the child.
