Documentation of Level 2 screening clearance from DCF was either incomplete or missing for the operator, program personnel, volunteers or substitutes. (Section 19.5, number 1) Record Keeping [SR]
Correction / follow-up note
One personnel that was present was missing their Level 2 background screening clearance from DCF. Technical assistance was provided with the Site Coordinator and also the Program Coordinator regarding the required documents. This noncompliance was corrected during the inspection with the personnel leaving once the staff to child ratio had been met.
