Correction / follow-up note
Provider did not have a file for employee listed on the supplemental. Provider stated she had been at the facility working since 11/10/25. Employee attempted to reach her supervisor AF, but AF failed to respond to provider while licensing was present. Corrective Action: Provider is to email a copy of employee's file to licensing by 01/07/2026. Provider will be closed from 12/19/2025- 01/06/26.
