Licensing Specialist provided technical assistance to the Director: Preschool child must have completed and signed Student Health Examination in their child file within 30 days of enrollment. The completed Student Health Examination is valid for two years from the completion date. A Student Health Examination must be provided by the due date for the facility to be in compliance.
