At the time of the onsite inspection visit documentation of Level 2 Clearinghouse screening clearance/ form 5131 and personnel file requirements documents was missing/ not available at the time of the onsite inspection visit. It was not onsite for review for childcare personnel (counselor), who was observed taking care of a group of 4th - 5th grade children. See supplemental notes for more information. Note: email notification received on 5/13/2026 from regional manager with the missing Level 2 screening as outlined in s. 435.04, for the above counselor. Technical assistance was provided.
