FINDINGS: A complaint inspection was conducted on 10/21/2025 and the following was observed; Four playpens and five infants were observed. Per director, there are 5 playpens, 4 in the classroom and one is kept in the hallway. After my walkthrough i did not see the 5th playpen. Then, the director was observed going to a vehicle and retrieving the 5th playpen. The director stated that she takes them home to clean over the weekend. - 3.6.4(B), Handbook: When napping or sleeping, young infants who are not able to roll over must be positioned on their backs and on a firm surface to reduce the risk of Sudden Infant Death Syndrome, unless an alternate position is authorized by a physician. Sleep sacks that fit according to manufacturers recommendations, do not restrict the infants arms, and will not slide up around the infants face may be used for the comfort of the sleeping infant; however, swaddling shall not be used unless authorized in writing by the childs physician. Written documentation of a physicians authorization must be on file at the facility. Documentation must include the childs name, childs date of birth, description of sleep position required, instructions for the use of any equipment needed, and length of time authorization is valid.
