All seats, safety restraint devices, and fixtures must be maintained free of rips and tears and in good repair at all times. Vehicles must be cleaned and sanitized routinely. The facility failed to maintain the vehicle as evidenced by [SEE COMMENTS].

The provider has been informed that the seatbelt cannot be used until it has been repaired. This will decrease the number of available seats from 13 to 12; no more than 12 children can be transported at a time. OLD VAN (from sister school) - the left side of the rear passenger doors could not open during the inspection, repair - repair broken door handle on back door - repair broken casing for speaker behind driver seat in first row * The older van cannot be used to transport children until the door has been repaired and can open fully.
