Sec. 7-45(b)(1), Ordinance: Vehicles must state the name of the child care facility on each side panel in six- to eight-inch letters. The back panel must have the wording Caution Transporting Children in a minimum of four-inch letters. The facilitys license number also must be printed on the side and back panels of the vehicle. The facility's vehicle(s) did not meet the requirement in that [see below].

Sec 7-30(a)(3), Ordinance: All child care personnel who provide care to infants at child care facilities shall, prior to caring for such infants, successfully complete the most current training course approved by the local licensing agency relating to providing an infant safe sleep environment, as identified on the local licensing agencys website. The owner of a child care facility must ensure that all such child care personnel are in compliance with the training requirements of this section, and that such training is renewed every 2 years, in the same manner as provided for the initial training. Successful completion of the required training must be evidenced by a certificate, or any equivalent documentation, issued by the applicable training organization in the name of the individual who completed the course and indicating the date the course was completed. Complainant alleges that on 4/3/2024 two children, EB and NM went outside and an outsider brought them back to the school. The doors do not have locks to keep them inside. Statements from the teachers (KL and RS) and Director (MD) were received. The Director initially claimed that she was not aware of any incident involving children leaving the facility and nothing happened on 4/3/2024. After speaking with the teachers, it has been confirmed that a lack of direct supervision occurred on 4/3/2024 and the Director was informed of the incident.
