Sec. 7-33(e), Ordinance: There shall be at least one child care staff member providing direct supervision at all times when one or more children are present. At no time shall any children be left alone for any reason without direct supervision by a child care staff member. Direct supervision means watching and directing the childrens activities within close proximity in the same room or within a designated outdoor play area, and responding to each childs needs. Child care personnel and volunteers at a facility must be assigned to provide care to a specific group of children and be present with that group at all times during the day, including during meals, napping, and transportation of children 2.4.1(B), Handbook: Children must never be left inside or outside the facility, in a vehicle, or at a field trip location by themselves. During the course of a complaint inspection conducted on 12/3/2024 the following non-compliance was observed; On 12/2/2024, the infant teacher was observed taking four infants out to the playground one or two at a time. The first child was left unsupervised as she retuned inside to get a second child. Then, two children were left unsupervised as she went back inside to retrieve the last two children. She left the infants alone a third time to retrieve a blanket. TA was given to the preschool about better transitioning into the playground. The director stated that she already informed the infant teacher to use the emergency crib to take them to the playground all at the same time.

A complaint allegation was received alleging that CCLE licensing specialist J. Thyme was conducting a routine inspection at near by facility and overheard a scream/cry for help from a young child. Upon hearing the loud noise coming from across the street, the CCLE specialist (J. Thyme) witnessed an infant 11 months, being restrained by a teacher. The child was picked up and roughly put onto the ground and the staff member continued to hold the child down roughly. J. Thyme. (CCLE) then approached the director regarding what she witnessed and reported to her that the Child Abuse Hotline will be notified and that the parent would need to be called. Davila pick up child S. from the daycare, she had bites marks and bruises on her left arm begging from the top of the palm all the way around and to the top of elbow (front and back) also she had a bit mark on her cheek. Bruises on her nose, top of the lip, bottom of the lip. Skin tear near the bottom of her left eye. Her right eye was red at the bottom. On October 14.2024 Childcare Licensing received a complaint alleging multiple bites marks and bruising on left arm, top of palm to elbow front and back, and bottom of lip and left eye. During the onsite investigation visit on October 17, 2024, the following corrective action was completed at the time of the Complaint visit: Two broken play structure, one with rusty and sharp screws exposed/protruding was accessible to the children. Both play structure were removed from the playground during the complaint visit. Note: Provider was advised to seal or cover areas of the fence that are jagged and accessible to the children.
