The attendance sheet/class roster must accompany the child care personnel and the group of children throughout the day should they leave the classroom. In order to account for childrens whereabouts, children who are reassigned to different classrooms throughout the day should be signed in and/or out of the rosters for those respective classrooms each time. During todays visit, provider was not able to show proof of a current vehicle inspection. Provider requested a copy from the insurance company, which was submitted to the license monitor via email. This area was corrected the day of the inspection. Complaint alleges that a teacher got bitten by a dog on Friday during pet week. Child is terrified by what he/she says they saw. A dog was inside the school. There is concern about vaccination and allergies that children may have and be exposed to. Parents are not notified or asked for a written consent. FINDINGS: A statement from the director and teacher was received. Per director, it was take your pet to school day. She was the only one who brought her pet to school. She was asked if there was a schedule for the activity and she stated that it was from 10 to 11 but that the dog was in the building all day because she was still at work. The preschool owns a 15 passenger van that has a total of 13 seatbelts available for children. On the date of inspection, only 13 children were picked up from the elementary school.
