The following hazard was observed in the outdoor play space [When transitioning from concrete to mulch it was found that the concrete had developed cracks and broken pieces along the edges at two corners. ]. - When coming out the door leading to the playground and walking on the concrete, a section can be seen with multiple concrete cracks. This is very apparent were the concrete meets the mulch and broken pieces along the edges were present at two corners. This is due to the cement having no support. This inspection was signed by the provider on 2/3/26 and revised on 2/5/26.
