- During the complaint inspection, observed video footage provided by the quality assurance director of the teacher in the Caterpillars class lifting a child by the arm inappropriately from napping on the cot. The cot was placed between the classroom child-sized table and the childrens bookshelf, likely leading to the teacher being unable to assist the child properly up from nap. This resulted in a hazard. A few seconds later, the child (victim) and another male child wearing a gold/yellow shirt came down the slide. The child (victim) was the first in the group to come down the slide. He appears to hop or jump into the sliding position as he slid down the slide quickly. The male child behind him, began to slide down the slide and abruptly turned backwards on his way down. - Per the director of the facility, mulch is under the turf surrounding the play equipment. However, the turf covering the area where student N.G fell was removed. Approximately 9 inches of compact dirt was measured under the turf. When other areas of the playground under said play equipment were observed, mulch was mixed with the dirt. No specifications of the fall zone were presented to the Environmental Specialist II. The child fell off the slide onto the ground and subsequently broke his femur. Please see advised that mixture of dirt and mulch is not an approved resilient surfacing material

- The incident occurred on October 16, 2025. The Department was not informed until October 20, 2025. There is no evidence that the provider notified DCF. Technical assistance was provided in that any unusual incident involving a child which may reasonably construed to constitute abuse and/or neglect shall be reported to the abuse hotline immediately, and to DOH within 24 hours of occurring. Also, technical assistance was given to properly document detail surrounding all incidents.

- On 8/30/23 a child had their finger caught in a door while following the teacher. The child was provided ice but no incident report was created. The child told their parent and the parent spoke to the assistant director on 8/31/23. The parent sent a follow up message on class dojo that they would like to be informed when and if any thing occurs in the future. Technical Assistance: Ensure any unusual incidents/ accidents are correctly and accurately logged on an incident from regardless of the severity. Parents should be notified the same day and acknowledge they have a received the form by signing it.
