- CCRC observed the resilient ground cover in the toddlers and the 3-4 playgrounds had wholes and cracks. Wholes varies with the size and depth. These whole are trip hazards. CCRC observed that one crack is a foot long, the cover was lifting up. CCRC observed that the area underneath the crack was filled with foam sealant, The crack was easy to open exposing the sealant. TA was Provided: Owner inform CCRC that the are aware of this issue and they working to solve these problem. Provider is given 30 days to bring this standard into compliance. 3.5 Outdoor Play Area A.
