Correction / follow-up note
The Licesning Speclist received notification that the facility had a change of Director on 8/5/25. The Licensing Specialist was not notified until 10/13/25. This standard has been completed at the time of the inspection as the provider notified the Licensing Specialist in an email and will provide the requested documents to make the change for the facility. Per the DCF Handbook Child care facility owners must notify the licensing authority within five working days of when the facility loses a credentialed director or when there is a change of director. 1.
