The personnel/ volunteer (ten hours or more per month) record did not include a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. CCF Handbook, Section 7.4, C
Correction / follow-up note
During the time of the inspection the Licensing Specialist monitored 5 staff files with an expired Child Abuse and Neglect reporting form. This was corrected at the time of the inspection as all 5 staff signed a new form dated 10/3/25. Per the DCF Handbook CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel.

Correction / follow-up note
During the time of the inspection the Licensing Specialist observed the diaper changing table mat was cleaned and sanitized after being used in the Toddler 1 classroom. The changing table had dirt and crumbs under the green mat on the changing table. This was corrected at the time of the inspection as the teacher in the classroom cleaned and sanitized under the mat during the time of the visit. Per the DCF Handbook E. When children in diapers are in care, there must be a diaper changing area with an impermeable surface that is cleaned and sanitized or disinfected after each use.
