- Incident occurred on June 5, 2024 in the morning. Provider self-reported the incident to licensing in the afternoon of June 5, 2024. Based on footage provided by the provider, a two-year-old child was left behind on the playground during the transition time between drop -off and the beginning of the school day. The footage showed the child went to the child designated classroom door, but the group was not in the classroom they were at the gym as part of the transition from drop off and coming into the building from the playground.
